Insurance/Fees

Elizabeth accepts the following insurance:

Wellmark Blue Cross Blue Shield, HealthPartners, Midland’s Choice, Iowa Medicaid (Wellpoint & Iowa Total Care), and Aetna.

Each client is responsible for knowing their own insurance coverage. You are encouraged to contact your health insurance company before engaging in services. Although Elizabeth is credentialed with the above insurance providers, some plans may vary and it is important to confirm that Elizabeth is a covered provider under your specific plan.

Private Pay Rates are as follows
